Investigation Summary
On November 9, 2010, Employee #1, a truck driver of Wagram Paper Stock, Incorporated, was performing his normal duties. He had hooked his truck to a trailer and had jacked the landing gear up onto the trailer as well. He walked into the yard, where other employees were operating forklifts to move bales of paper. It was unclear, but it was suspected that he may have tripped and fell, resulting in him being run over by the right front wheels of a forklift, which resulted in killing him.
