Investigation Summary
At approximately 2:00 p.m. on February. 6, 2013, an employee was inside an excavation tying in vertical rebar to the foundation rebar cage. Meanwhile, a coworker removed a bracing to the rebar cage at one corner. This caused the entire rebar wall to collapse rapidly in a domino motion. As a result of the incident, the employee sustained bruise and swelling to both of his legs, however, he was not hospitalized
