Investigation Summary

Investigation Nr: 202544235
Event: 04/13/2012
Employee Injured at Security Gate

On April 13, 2012, Employee #1 a security officer, was stationed at the warehouse gate, which leads to the airport. Employee #1's responsibilities included but not limited to checking the identification of the personnel entering and exiting the warehouse. On day of the accident, two coworkers were transporting a tug tow tractor with three dollies. The three dollies had empty unit load devices attached behind the tractor. As the coworker of the tug approached the warehouse gate, he slowed down to show ID badges to Employee #1. However, the coworker did not make a complete stop at the exit gate and did not ensure that the tug and the three dollies were safely clear out of the warehouse prior to increasing the speed. As the coworker, sped up, the third dolly hit the yellow safety pole located on the right side of the gate. This caused the third dolly to swing from left to right and struck Employee #1 standing on the left side. As a result of the accident the Employee #1 sustained serious injuries and trauma to his legs, and was subsequently hospitalized for several days for contusions, bruising and abrasions suffered as a result of this accident.
