Investigation Summary
On March 29, 2012, Employee #1 amputated the tip of his left middle finger when using a Roper Whitney Corp Pexto Metal shear to cut phelonic pieces. The shear guard had been removed for some time from the shear and was unguarded at the time of his accident. He took the tip of his finger with him to St. John's hospital where they reattached it to his finger.
