Event: 06/11/2011
Employee's Thumb Is Crushed by I-Beam

At approximately 2:05 p.m. on June 11, 2011, Employee #1 was working as a project superintendent for a general contractor and construction management company specializing in both commercial and residential projects. Employee #1's responsibilities as a project superintendent included supervision of the project and subcontractors. That day, Employee #1 was working on the roof level of a building under construction. Employee #1 and Coworker #1 were loading pieces of I-beam into a metal trash bin. The I-beam pieces measured approximately three to four feet long and approximately 12 inches wide. While loading the I-beam pieces into the trash can, Employee #1's left thumb became pinched between the I-beam and the trash bin, and the thumb was crushed.
