Employee Suffers Fractures after Falling Off A-Frame Ladder

On January 18, 2011 Employee #1 started working on that day at 12:00 a.m. and the accident occurred at 7:00 a.m.. The thermal test in chamber C-7 required several days to complete, and the accident occurred on the second day of the test. Liquid nitrogen was used to cool parts inside chamber to -300 F degrees, and a heater inside the chamber to heat parts to +300 F degrees. Prior to the accident occurring, Employee #1 observed some nitrogen gas leak from the top of chamber, and all 3 clamps at the top of chamber were not secured down. After confirming with the test engineer to bring chamber back to room temperature, and all 3 clamps needed to be secure at the top; Employee #1 used 8 foot A-frame ladder to access clamps at the top of chamber. Employee #1 fell off ladder while he was standing on the 6th rung of ladder. Employee #1 did not remember anything about the accident and why he fell. There were no actual witnesses to the accident. Employee #1 was taken to Centinela Hospital and was hospitalized for 3 days. Employee #1 suffered broken right wrist and shoulder pain.
