Investigation Summary
At approximately 8:30 a.m. on February 12, 2011, an employee and a coworker were performing light bulb and ballast replacement on a paint booth. As they completed the work and while exiting, the employee stepped off the unprotected catwalk back onto the paint booth's filter area. He fell through the filter to the floor approximately 16 feet below. No fall protection was provided. The employee was transported to Galesburg Cottage Hospital and later air lifted to OSF Medical Center In Peoria, Illinois, where he was pronounced dead at 2:20 a.m. on February 13, 2011. The preliminary cause of death was head trauma injuries. The incident occurred inside the plant so no adverse weather conditions were present.
