Investigation Summary
At approximately 12:30 p.m. on May 3, 2012 Employee #1, a commercial driver of Betts Express sustained fractures to his left clavicle and three ribs when he fell approximately three (3) feet from the v-bar of his Bottom Dump Truck at a jobsite. At the time of the accident, Employee #1 was delivering a load at a jobsite, when his truck's front driver side fender became loose. At his first stop, Employee #1 placed the fender in the rear trailer. At his second stop, Employee #1 decided to place the fender in the front trailer. Although both trailers were empty, the front trailer experiences less vibration than the rear trailer and would be less likely to damage the fender. He climbed up approximately three (3) feet onto the V-bar which is located between the two bottom dump trailers. For an unknown reason, Employee #1 fell from the V-bar as he was walking toward the steps located on the rear of the front bottom dump trailer.
