Investigation Summary
At approximately 8:49 a.m. on May 19, 2012, Employee #1, a cement mason journeyman with Beta Construction Inc., was cutting wood pieces to create forms for concrete. During this task, Employee #1 secured the work piece with his left hand and operated the hand saw with his right hand. Employee #1 was sawing off a 12 in. section of a 2 in. by 4 in. piece of wood when the portable circular hand saw he was operating experienced a kickback reaction. The work piece was not secured to a stable platform during the process. The guard did not fully activate which resulted in the amputation of the employee's left thumb. Employee #1 was hospitalized.
