Investigation Summary
At 3:30 p.m. on February 7, 2012, an employee was rigging a 60 inch drilling tool on his trailer in preparation for unloading. The drilling tool had been loaded on its side in San Diego, CA and transported to Paso Robles, CA equipment yard. The employee attempted to place a chain through the bottom of the drilling tool, and the bottom door became unlatched and fell downward, landing on the employee's left foot and ankle. The employee was transported to the hospital. The employer notified the Division of the incident on at 4:00 p.m. on February 7. Specifications of the spin bottom drilling tool were width: 60 inches, height: 48 inches, weight: 3,000 lbs. approximately. There was a Hyster powered industrial truck being used to assist in unloading the drill bit: Model Number: V2STGSIDESHIFT (H155XL), Serial Number: F006A06970N, 15,000 lb rated capacity. The employer was a caisson drilling and footing drilling company contractor.
