Investigation Summary
At approximately 12:30 p.m. on May 24, 2011, Employee #1 was transporting medical records to the "C" yard of a correction and rehabilitation compound. He opened "C" yard gate and proceeded through it. As he closed the gate, the lower portion of the gate-locking device struck his left middle finger. Employee #1 sustained a laceration on this finger. He was transported to a regional hospital and was then referred to another medical center, where he was treated and released.
