Investigation Summary
At approximately 6:24 a.m. on April 25, 2011, Employee #1 was working as an installer and electrician for a company that designs and builds high-performing solar energy systems. He was working alone on a 14-foot fixed ladder, performing his daily duties. This ladder was not equipped with grab bars or any other safety device. Employee #1 descended the ladder, and fell, and sustained lacerations and abrasions to his back. There were no witnesses to the accident. Employee #1 was transported to Providence Holy Cross Mission Hills Hospital, where he was admitted for five days. The Los Angeles Fire Department notified the OSH Division of the accident on April 25, 2011. The OSH Division then initiated an investigation of the accident on May 10, 2011.
