Investigation Summary
At approximately 7:54 p.m. on January 5, 2011, Employee #1, a warehouse specialist, was driving a Crown pallet jack with a load of paper down the main aisle, and he was struck by a coworker, who was operating Crown Order Picker loaded with four empty pallets.
