On August 24, 2011, an employee was conducting an adjustment on a Mazak slant 60 CNC lathe. The employee was in back of the lathe making the adjustment while the machine was energized. The employee was struck by the cable chain, pinning his head against a horizontal piece of angle iron. The employee was killed.
