Employee Is Injured When Pinned Between Forklift and Table

At approximately 8:40 a.m. on August 7, 2013, an employee, operating pallet jack, Equipment Number 39407, Model Number PE3540-60, approached dock Door#1. He started to make a turn so that he could line up with the dock door. The employee started to brake but the pallet jack did not stop due to a leaking brake seal and kept rolling forward. The pallet jack continued forward and struck a metal table that was mounted on two concrete barrier posts next to dock Door #1. The employee's right leg was pinned between the pallet Jack and the metal table, causing a fracture to the right femur. The employee was hospitalized and treated for his injuries.
