Investigation Summary

On March 3, 2012, an accident occurred at J L Furnishings located in Compton CA resulting in an amputation of the ring finger on the left hand of EE#1. The employer didn't report the injury to DOSH. High Hazard received a report of the injury from DWC on November 19, 2012 and the inspection was opened on January 7, 2013. EE#1 was making borings into a structural member of a chair using a Grizzly Horizontal Boring Machine (Model G4185 S/N 05240). EE#1 had bored sections of a chair that required a 3" depth and had adjusted the clamp to accommodate the thickness of the stock. EE#1 then made preparations to bore into another section for the chair that was only 1" thick. He neglected to adjust the air powered clamp for the narrower width of the stock. When EE#1 stepped on the foot pedal to actuate the boring process the clamp came down on top of his finger causing an amputation of the ring finger on his left hand. J L Furnishings is a furniture manufacturer who specializes in making Hotel Furniture and employs approximately 100 employees. At the time of the accident, J L Furnishings operated at 3040 E. Maria St. Compton CA 90221; however, since the accident, the employer now manufactures from their facility located at 19007 Reyes Ave Rancho Dominguez CA 90221. EE#1 was a full-time hourly employee. EE#1 was working alone at the time of the accident and there were no witnesses. It was determined that the injury occurred due to EE#1 placing his finger in the zone of danger from the descending air powered clamp which caused the amputation injury. EE#1 was transported to the Downey Surgical Medical Center for treatment. The employer failed to report the serious accident.
