Investigation Summary
At approximately 9:30 a.m. On September 3, 2012, Employee #1, a 34-year-old male of Unified Grocers Inc., was operating a Yale stand-up forklift number 8027 and a coworker was operating a Yale stand-up forklift number 7377. Both Employee #1 and the coworker failed to follow established traffic procedures and the two forklifts collided at the intersection of isle fifty-one and section-N. Employee #1's right leg was outside of the operator's compartment and got caught between the two forklifts as they collided. Employee #1 suffered a crushing injury to his right lower leg resulting in an amputation. Employee #1 was transported to USC Medical Center for treatment and then transported to Kaiser Permanente Hospital. Unified Grocers Inc., was engaged in the warehousing and transportation of packaged food items and was located in Commerce, California.
