Summary Nr: 202561015Event: 07/29/2010Employee Is Electrocuted
On July 29, 2010, Employee #1, an electrician and the sole owner of an electric company, was changing out the electric meters from the inside of two trailers electric service box. The employee was mounting new electric meters on the top of the electric service box for the trailers. He was using a steel wrench on an energized circuit. Employee #1 was electrocuted and was hospitalized. The city electric company tested the circuits in the panel and found them to be energized at 119 and 117 volts, each on a 15 ampere circuit. The main cause of this injury was the lack of Lockout-Tagout Procedures being implemented. The circuit breaker panel (Box Number 2) was to be disconnected at the electric power drop from the city's power supply transformer but that circuit breaker was in the closed (energized) position, when the responders arrived at the scene. The city electric company disconnected their power service from the power drop of the facility by disconnecting their transformer from the power drop.
