Investigation Summary
On July 16, 2010, Employee #1, an operational manager, was pruning a palm tree, approximately 24 ft tall. He was on a 28 ft metal extension ladder and was using a hand held tree trimming saw. He had placed the ladders feet on sand and leaned it against the round truck of the palm tree. The ladder was found on top of the hedges that surrounded the tree. The tree trimming tool was still in the tree hung on the limb in which Employee #1 was attempting to cut. Employee #1 fell to the parking lot on the other side of the hedges. He was hospitalized and later pronounced dead.
