Investigation Summary
On June 24, 2010, Employee #1 was draining oil from an automobile with a fluid evacuation system. While draining the oil, an explosion occurred in the vehicles upper tank, causing the tank to shatter. Fragments of the tank struck Employee #1 in the neck and chest. Employee #1 was taken to a nearby medical center, where he was treated for his injuries and released.
