Investigation Summary
At approximately 10:55 a.m. on August 3, 2010, approximately 240 ft of rural water pipe was installed. Employee #1 was installing the next section of water pipe in the excavation and the owner, was in the excavation pushing on the end of the pipe to seat the pipe into place. The west trench wall where Employee #1 was working, caved in on him and, he died as a result of the accident.
