Investigation Summary
On June 25, 2010, Employee #1 was installing baffle shoots within the trusses of a residential building for the preparation of spray foam insulation. He conducted the installation from an aluminum light duty scaffold with an approximately 28 in. base width and a 12 ft high platform. During his work, he lost his balance and fell, striking the floor and losing consciousness. He remained on life support until his death three days later.
