Investigation Summary
On or about July 30, 2010, Employee #1, an electrical test technician, was leaning over or downward to place completed pass/fail inspection reports in plastic bags. The plastic bags were attached to the outside of the reels, near the arbor holes of wire which had previously undergone an, 8,000 volt, AC Voltage Withstand Test. His head came in contact with the tip of an ungrounded conductor and insulation compounds. Employee #1 suffered a fatal electric shock. The path to ground was the employee's head through his left foot.
