Employee Is Killed in Operational Machine

At approximately 2:45 p.m. on March 30, 2011, Employee #1 was retrieving bundles of recyclables and dumping the material into the hopper of a Morbark 727 trommel screen machine. The machine was approximately 45 ft in length and had an operating height of approximately 14 ft. The trommel was used to separate glass from desired recyclables such as paper and plastic. The machine was hydraulical powered with an electric motor starter. An operator would occasionally need to enter the trommel drum to remove jammed material in belts, screens, and the infeed hopper. Ladders were required to access both points of entry on the trommel, which were in excess of 4 ft in height. While Employee #1 was operating the trommel, it jammed. He decided to unjam the machine without isolating and locking it out. He entered the trommel. As he unjammed the machine, it started up. The moving debris and machine parts killed him. The supervisor turned off the machine and found his body in its drum.
