Employee Amputates Finger in Die Cutting Machine

On June 17, 2010, Employee #1, a machine operator, was manually feeding a 23 in. by 31 in. piece of corrugated cardboard into a 50 in. die cutting machine. Employee #1 was holding down the cardboard on the feed table when, for some reason, the speed in which the material was being fed pulled in the machine operator's left hand. Employee #1 amputated a finger on his left hand. Employee #1 was hospitalized. Upon further investigation, it was determined that the fixed guard was improperly installed.
