Investigation Summary
On April 19, 2012, Employee #1, with Abacus Construction Inc., was working from a platform which was elevated at least 14 ft off the ground by a Lull (Model 844) variable reach rough-terrain lift. Employee #1 was not wearing a personal fall arrest/restraint system or positioning device. For unknown reasons, Employee #1 fell from the work platform causing injuries to his head, face and brain. Employee #2, a coworker, heard but did not witness the fall. Employee #2 responded to Employee #1 who was transported via ambulance to a local hospital where he was admitted and treated for multiple injuries including a serious closed-head injury. Employee #1 remained in the hospital for the next five days until being released.
