Investigation Summary

Event: 04/06/2012
Employee Amputates Two Fingers on Saw

At approximately 12:00 a.m. on April 6, 2012, Employee #1 was cleaning sawdust from a multihead resaw, described as a Wood-Mizer industrial saw. The sawdust had accumulated in an area on the saw, where it could be removed through an unguarded opening in the cover of one of the saw blades. Employee #1 was removing the sawdust through the cover opening as the saw was running. He had been instructed to do so, because the conveyor system which normally removes the sawdust from the machine was not working. While cleaning out the sawdust with his left hand, two of his finger, one being his pointer finger, contacted the rotating saw blade. The saw blade amputated these fingers. He was transported to a medical center. At the medical center, Employee #1 underwent surgical procedures and was hospitalized for postoperative care.
