Investigation Summary
On September 6, 2011 a carpenter with Mid-State Builders, Inc. of Citrus Heights was working alone on a 2-story residential home. Mid-State Builders, Inc. was doing repair work on the home as a result of a fire. Employee #1 had been assigned to do much of the work which included window replacement and checking the roof. Employee # 1 was working on the second level of the metal scaffold erected by the plastering sub-contractor. Employee # 1 confirmed the working platform had only two planks instead of three that would have made it a fully planked area. Employee # 1 was standing on the planks removing roof tiles when the accident happened but no one knows how he fell off the scaffolding. Employee # 1 belief that he fell through the gap associated with not having three planks. The Employee # 1 sustained fractures to his rib, neck, and brain and hearing loss in his left ear. Employee # 1 was transported to Kaiser South hospital in Sacramento, and he was hospitalized for seven days before being transferred to a rehab facility for two weeks.
