Employee's Fingertip Is Crushed by Object and Is Amputated

On September 10, 2011, Employee #1 was securing the hitch of a trailer dolly to a tractor for transport. He used a forklift to lift the dolly into place and at a position, where the hitch was located over the tractor's receiver. Employee #1 then lowered the forks of the lift to release the tension on the dolly. Next, he inserted a hitch pin, when the hitch slipped off the receiver. The receiver fell approximately 6 in., crushing his right middle finger upon the lift's fork. His supervisor transported him to a local hospital. At the hospital, an emergency room physician removed the crushed bone in his fingertip and then repaired the damaged tissue. Employee #1 was released following the surgical procedures. He returned to work in a modified capacity on his next scheduled workday.
