Investigation Summary
On March 19, 2011, Employee #1, an order clerk for a warehouse, was filling orders in the distribution warehouse. The non-powered roller used to connect two lines of conveyor belt became disoriented, falling in-between the two belts. Employee #1 reached his hand up from the bottom of the system and tried to dislodge the roller. His finger became caught between the roller and the moving belt, amputating it. He was not hospitalized for his injury. The employer had a lock-out/tag-out program, and Employee #1 had been trained appropriately.
