Employee Seriously Injured from Pallet Jack

On February 25, 2011, Employee #1 was picking merchandise ina store room, the pallet jack was positioned so Employee #1 could not remove the necessary product. Standing on the floor, Employee #1 began to move the pallet jack towards him, when the handle abruptly jerked to his left, and into metal racking. Employee #1's left ankle was pinned between the pallet jack and the base of a metal rack, which also included the pallet jack coming onto his foot. Employee #1 sustained a shattered left ankle with a large hole just above the ankle. Surgery was performed to insert a plate with screws into the ankle and to sew up the hole.
