Investigation Summary
On May 19, 2010, Employee #1 was working on the HVAC system in the ceiling, while standing on a 6-foot, A-frame ladder. Employee #1 stepped on top of the ladder to reach a valve, when the ladder wobbled and caused him to fall from it. Employee#1 struck his head on the adjacent counter and he was hospitalized.
