Investigation Summary
On August 2, 2010, Employee #1, a plastic fabricator, was using a table saw to rip very thin strips of plastic from a blank. The employee had set up a fence and used a push stick, but the blank kicked back about half-way through the cut drawing. The employee was grabbing the thin strip with his left hand, and his left thumb was pulled into the revolving blade. Employee #1 underwent surgery to have his thumb reattached and was hospitalized for seven days. The blade guard had been pushed upward to get it out of the way.
