Investigation Summary
According to the employer rep statement, the roofing crew had just finished a morning meeting when the crew was starting to work on replacing roofing material on a roof. An employee was working near the edge of the roof with his back to the edge. He was bent over cutting roofing material, he stood up, stepped back and tripped over the parapet and fell to ground. He was hospitalized for a day and died on May 3, 2011.
