Employee Falls from Extension Ladder

On May 29 2010 Employee #1 was attempting to repair a door at Fire Department. While standing on the extension ladder, he fell from the ladder. The height was approximately nine feet from the cement floor. Employee #1 suffered head injuries. Employee #1 was taken to a hospital and died the next day.
