Employee Is Injured in Fall from Ladder

On June 21, 2010, an employee fell from a ladder while transitioning to an unsecured work platform set on mismatched ladder jacks at a height of approximately 15 feet. The employee sustained serious injuries including fractures to both heels, his right wrist and left occipital The employee was hospitalized and treated for his injuries. The scaffold was not constructed or inspected by a competent person. Also, the employee was not trained in the proper use of a ladder jack scaffold which requires the use of a personal fall arrest system.
