Employee Crushed in Forklift Accident

On May 18, 2010 Employee #1 engaged in the removal and replacement of an electric motor of the planer mill at the facility. The motor was being hoisted into position by forklift with a hoisting bar attached to the forks. As the motor and the hoisting bar was being maneuvered into position to mount the electric motor on its base, the hoisting bar became obstructed by nearby support structured. When the hoisting bar broke free from the obstruction, the bar and motor abruptly jumped in an upward direction, crushing Employee #1's head between the hoisted motor and the nearby support structures.
