Investigation Summary
Employee #1 and a coworker were on their way back to the office, after making several deliveries. The coworker stated that Employee #1, climbed into the sleeping quarters of the tractor trailer at approximately 3:30 p.m. The coworker continued driving back to the office, as Employee #1 was still sleeping. The dispatcher went out to the truck to ensure that Employee #1 was ready to roll with the next distribution load, when he got no answer from the sleeping cabin. The dispatcher obtained the spare keys to the truck and found Employee #1 still in a state of what looked like sleep. The dispatcher could not get Employee #1 to respond, and called emergency response as well as management, to inform them of the situation. Once EMS arrived and assessed Employee #1's condition, he was pronounced dead at the scene.
