Investigation Summary

At approximately 8:00 a.m. on December 26, 2012 An incident occurred while Employee #1 was in the back of the van, standing on the diamond grid metal floor, Employee #1 turned and began to walk from one shelf to another with a package in his hand. Employee #1 slipped on the floor because moisture from condensation from early morning dew. The momentum and direction from the fall within the truck carried Employee #1 out the back of the truck, falling onto the warehouse concrete floor. Employee #1 fractured his ankle in three places. Employee #1 stayed in the hospital for over a day and released.
