Investigation Summary
On December 17, 2014, Employee #1, a 30-year-old male with Action Metal Recycling Inc., was trying to forcefully bring down the cab of a yard tractor called a "goat." The cab then abruptly came down crushing Employee #1's head between the fixed 4 in. by 4 in. metal bar/metal guard and the upper portion of the cab of the goat. Employee #1 was killed in the event.
