Employee Seriously Injurs Finger in Press

At approximately 12:30 p.m. on October 6, 2011, Employee #1 was working on a beverage can assembly line, inside a manufacturing plant. Employee #1 was attempting to remove an object that was jammed inside the domer die, by opening the guard door and reaching in with his left hand, to remove the obstruction. As Employee #1 was reaching into the domer die to remove an obstruction with his left hand, when the machine accidently went into auto start mode. Once the auto start mode was activated, the mechanical punch press went into cycle, and the punch came down onto the domer die. Employee #1 still had his left hand inside, and the punch crushed the top portion of his left index finger as it struck the domer die. Employee #1 was transported to a local medical center for immediate medical care. Employee #1 was transported by ambulance to a local hospital for surgery. Employee #1's top half of his left index finger was amputated, and Employee #1 spent three days at the hospital, then was released for outpatient care.
