Employee Falls Through Skylight and Suffers Injuries

On September 14, 2011, Employee #1 was laying pipes for a steam cleaner in an attic space. The attic space was located above the production floor and had a skylight. The skylight was not guarded or covered. Employee #1 carried the pipes through an access hatch. He then dragged them towards their location while walking backwards. As he was backing up to set them down, he stepped onto the skylight and fell through it. Employee #1 fell approximately 15 ft and struck a concrete floor. He suffered unspecified shoulder, back and head injuries. Emergency medical personnel were summoned. They arrived and transported him to a medical center. Employee #1 underwent surgical procedures and was hospitalized for postoperative care.
