Employee's Forearm and Wrist Lacerated in Saw

At approximately 6:35 a.m. on June 29, 2011, Employee # was instructed by his supervisor to report to a coworker where he was given a new job assignment of cutting scrap aluminum extrusion into smaller pieces for recycling. The coworker set up a 10-in. compound slide saw, a Hitachi Model Number C10FSH, on a sheet of plywood that was placed on a cart with wheels. The saw was not secured to the sheet of plywood and the plywood was not secured to the cart. The wheels did not lock to prevent it from walking or moving from its location. The coworker stated that he took approximately 10 minutes to explain and demonstrate the use of the saw to Employee #1 and then walked away. Employee #1 gathered the scrap aluminum extrusion into a pile and then started to cut them into smaller pieces. He proceeded by placing a piece of aluminum extrusion without securing it to the saws vise assembly and started to cut it. The cart moved and the material being cut was displaced. The saw kicked the material up and to the left, forcing Employee #1's left forearm and wrist into the moving saw blade. Employee #1 suffered severe lacerations to his left forearm, wrist, and the associated muscles, tendons, and nerves that control the extension and abduction movements of his left thumb. Employee #1 was hospitalized.
