Investigation Summary
On March 30, 2011, Employee #1 was operating an electric forklift. As he dismounted, his right index finger was caught in between a grooved portion of the lift bracket and the movable forks of the forklift. The pinch point amputated his finger. He was transported to a medical center, where he underwent surgical procedures and received postoperative treatment and was then hospitalized.
