Employee Injured Using Forklift

At approximately 9:30 a.m. on July 22, 2010, Employee #1, a forklift operator working for a contractor, was preparing to operate a forklift. Employee #1 was preparing to inspect a company forklift for operation, proceeding to the rear of the forklift, when the forklift moved towards him, and he stumbled, backwards to avoid the forklift. When Employee #1 fell backwards, he struck his head against the pavement. Employee was taken to the hospital for observation.
