Investigation Summary
On August 12, 2010, Employee #1 was assigned to install roof sheathing on a barrel roof. He leaned out between the mid-rail and top rail of a guard rail and tried to nail the roof sheathing on the edge of the barrel roof. He lost his balance and fell 17.75 ft, sustaining wrist, rib and vertebrae fractures. Employee #1 was hospitalized.
