Investigation Summary
At approximately 6:20 a.m. on June 11, 2010, Employee #1 was in the process of greasing the Belvac CC93 Body Maker Trimmer. While Employee #1 was greasing fittings in the vicinity of the belt and pulley, his left hand became entangled in the belt and pulley causing the following injuries: Degloving of the left hand index and ring fingers and bone injuries to the left hand middle finger, requiring amputation of the finger tip. Employee #1 was wearing gloves at the time of the accident. The guards were removed to perform the greasing operations and the machine was running at the time of the accident.
