Employee Crushed Under Forklift

Employee #1 was underneath a forklift truck model, engaged in removing the hydraulic cylinder from the upright assembly of the forklift truck. When Employee #1 removed the hydraulic cylinder, the removal of the cylinder caused a large change in the center of gravity and the forklift truck moved backwards coming off the jack stands, from under the vehicle. Employee #1 was crushed underneath the forklift truck. No additional blocks were placed under the counterweight or on the back wheels so that the truck can not fall backwards.
