Employee Is Injured in Fall from Roof, Later Dies

On July 6, 2011, Employee # 1 was working for his employer, a roofing contractor replacing the roofs on all the buildings at the apartment complex. Employee # 1 was in the process of installing insulation material near the southeast edge of Building Number six at the apartment complex. Employee #1 did not have any form of fall protection, and the safety monitor failed to notify the employee that he was too close to the edge. Employee #1 fell approximately 22 feet onto the concrete sidewalk. Employee #1 sustained extensive head trauma and was transported to Hillcrest hospital in Waco. He was hospitalized for a total of seven days, before he died. The company safety director reported the accident to the Fort Worth Area office on the same day and an accident investigation was opened on July 7, 2011.
