Investigation Summary
Employee #1, a machine operator for Stone Crafter Granite and Marble, Incorporated, was hit in the head by two falling slabs of granite. The owner of the company was operating the forklift when the left fork hit a slab, causing that slab and another slab of granite to fall. The two slabs of granite fell over and hit Employee #1 in the head, killing him. The investigation determined the forks of the forklift obstructed the owner's view because they were at eye level.
